  • A fever, sore throat or stomachache, or when you just don’t feel well.
  • Help with a minor cut or sprain.
  • A care plan for managing asthma, congestive heart failure, COPD, diabetes, high blood pressure or other chronic conditions.
  • Well-visit checkups or physicals for work, school or sports.
  • Prenatal or baby care visit.
  • COVID-19 or flu shot information.

Talking with all of your health care providers is key.

Think of your primary care team as trusted partners in your good health, there to help you every step of the way. Keep in touch through office visits, telehealth or the online portal. Ask questions and share information. It’s important to be honest about how things are going so they can help you come up with the plan that works best for you.

Your appointment may be with a doctor (M.D. or D.O.), physician assistant (PA) or nurse practitioner (NP). All three can diagnose and treat illness, prescribe medicine, order tests and help you feel better, so you’ll be in good hands.

The extended care team

A ChristianaCare CareVio ™ care coordinator may reach out to you by phone or text after your primary care visit to make sure you understand your care plan, help you refill prescriptions or help you schedule labs and tests. This is a call you will want to take!

Health care is so much more than medicine and hospitals. It’s about making sure you have a safe, warm place to live; healthy food to eat; help fighting addiction; support caring for your children; and a trusted partner to help you along the way. Our primary care team also includes behavioral (mental ) health experts, social workers and community health workers who can help you get the support you need to deal with life’s ups and downs that can get in the way of your best health.
